When inserting a rectangle or insertObjectAnnotation to my 512x512 int16 (grayscale) image (dicom) the pixel values are moved around

I import DICOM files from a directory and then i want to insert bounding boxes on those images. However, it appears that the pixels are shifted around when the grayscale to RGB conversion is performed. I have tried to convert the int16 to int8, double and so forth without success. The documentation for insertObjectAnnotation also claims that int16 is supported as input. I have also tried byteswap if it could be a endian problem ...
Is it any one out there with any suggestions?
