writing a .csv-File with certain format

Hello,
i have a Matrix and two vectors (x_axis and y_axis) and want to write them into a single .csv-File. The file should later look like the attached file. Is there a way to do this? If so, could someone help me out with this? Thank you for your effort.

To attach a file you should edit your question, the click the paperclip button, then both Choose file and Attach file buttons.
