How to output a table in stateflow

I am going to make a system about SAS(spectrum access system)-CBSD(Citizens broadband radio service device). Now I am struggled on how to make an output as a formula

Can you provide some more details on what you are trying to accomplish? For example, are you trying to output a MATLAB table from a Stateflow chart or a truth table? Also, what do you mean by "make an output as a formula"?
