i want to create an ATP form for a personal project
i want to have a checklist and open boxes to fill revisions of software and hardware
and save it at the end as pdf
where i need to start ?

MATLAB provides App Designer to create UI applications. You can add various components to your application from the Component Library like checkboxes, text areas, edit text and tables (useful for your application), according to your requirements. You can also define button callbacks to save the table data to pdf file as answered here.
